Get to the root cause of any problem with the 5 Whys technique — a systematic inquiry method that uncovers the real source of an issue by repeatedly asking "Why?" until the underlying cause becomes clear. This template structures the 5 Whys process on a shared canvas, making root cause analysis collaborative, visual, and auditable.

How to Use

1. Write the specific problem statement at the top using the Text tool — be precise; "things are slow" is too vague, "deployments take 4 hours" is workable
2. Ask "Why does this happen?" and capture the first-level cause as a sticky note or Card — connect it to the problem with the Connector tool
3. Continue asking "Why?" for each successive cause — add a new node for each answer and connect it to the previous one
4. When the chain reaches a cause that is systemic, structural, or policy-based, you've found a root cause — mark it in the root cause declaration section
5. For each root cause, create an action item in the bottom section with a clear corrective action and an assigned owner
